Welcome to EOU Student Health Center:                                                                          

 

Health care can be a concern for you when you are newly away from home, but Eastern Oregon University has a Student Health Center

(EOU SHC) on campus that is available for all your health care needs.   Our mission is to help you remain healthy, so you can attend classes and achieve your educational goals.   Confidential care is available, at the Student Health Center, to all Eastern Oregon University students who carry six (6) or more credits per term during the academic year (September-June).   There is no charge for individual illness related visits, as full time students are assessed a health fee that helps to fund the Student Health Center.   There may be costs to the student if diagnostic tests, medications or treatments are required to diagnose and treat your illnesses, or for physical exams and wellness visits. Students are encouraged to be informed of the health insurance plan they have while here at Eastern Oregon University.

Care is managed and provided by certified, licensed Nurse Practitioners, Monday through Friday eight hours per day.   Appointments are recommended and may be made by calling 962-3524.   The EOU Student Health Center is located on the corner of 6th and "L" Avenue,

La Grande Oregon.

An important issue when arriving on campus is your measles and mumps vaccination (MMR) history.  You will satisfy this requirement if, when you arrive on campus for fall term 2006, you have had one vaccination and receive the second prior to the start of winter 2007 term.  Remember, you MUST HAVE HAD ONE MMR VACCINATION prior to arriving on campus for the start of fall term.  You can receive the second vaccination required prior to the start of winter term in the student health center for a minimal fee. To prevent a delay in registration, please complete a certificate of immunization form (PDF file) and send it along with a copy of your vaccination records, to the EOU Student Health Center for review.     

 

Pre-participation sports physicals are required prior to intercollegiate sports participation and must be completed at the EOU Student Health Center.   When you arrive on campus, see the Athletic Trainer to schedule this exam.   The student athlete cost for this physical is $40. Cash or check payment will be due at the time of this physical exam.

These forms are located on SHC's web site and also available at SHC.   A Release of Medical Records form (PDF File) is available on our web site or at SHC for you to sign and return to the EOU SHC with your health history forms.   You must have this form completed prior to arriving for your exam, as we may need to order records from previous providers depending on your health history.

In addition, athletes need to be current on their tetanus vaccination.   Documentation of a booster tetanus injection within the last 10 years is required and must be provided to the EOU SHC before you will be allowed to participate in sports.   If you need a booster immunization, you may have this done for a fee at the time of your sports physical examination.
